My current plan on this is to let players chose between "Ranked" (or probably "Normal") and "Custom" before creating a new game. In ranked mode, the official ruleset will be used and skill will be enabled. As a result, the password, spectators and pauses options will be moved into custom options as well because ranked play is always expecting no password, one pause per player and spectators turned on. Custom skill limits will still be available for ranked play. "Send to next" will not be available for ranked play while "Send to random" will be available because everyone is free to decide whether to play it in ranked or not (this solves the discussion about skill being enabled for this mode or not).
In custom mode, the official ruleset will be used by default but skill will always be turned off. The host is free to alter all the settings or just leave them set to the defaults to simply play a normal game without skill being calculated.
The same is valid for survivor highscores. Also these will be available in ranked play only. Imho this is the most straight-forward implementation and the easiest to understand.
